Luke-Dorf Cooperative Agreement to Benefit Homeless Individuals (CABHI)—The Housing TEAM (Transition, Engagement and Mentoring) Program

Agency: Luke-Dorf, Inc.

RMC Research is evaluating a 3-year program implemented by Luke-Dorf, Inc., a mental health services provider in Oregon, and funded by the federal Substance Abuse and Mental Health Services Administration’s Center for Substance Abuse Treatment. The Cooperative Agreement to Benefit Homeless Individuals grant provides consumers with case management and referrals through Critical Time Intervention, and assistance finding permanent housing. Service coordination among county providers of housing, mental health, behavioral health, and other services is another key aspect of the grant. RMC Research conducts annual fidelity assessments of implementation of Critical Time Intervention and Permanent Supportive Housing and analysis of longitudinal outcomes related to housing retention, mental health and substance use symptoms, access to benefits, and contact with criminal justice systems.
